Athletic Forms & Participation Requirements

All students participating in sports at the junior or senior high level are required to have a physical examination every two years in ND, three years in MN, before they may participate in practices. This requirement is based on the state where the sport originates.

PCS Sports (Minnesota)

1. MN Physical Exam Form – required every 3 years (junior high included). For in-between years, complete the Annual Sports Health Questionnaire (included in #2).

2. MSHSL Athletic Eligibility Brochure, Eligibility Statement, and Health Questionnaire (read, completed and signed)

3. PCS Athletic Rules Sheet (signed and fees paid)

Minnesota State High School League (MSHSL) forms and addititional information can also be found on the MSHSL website.

Co-op Sports (North Dakota)

1. NDHSAA Physical Form – required every year (junior high included). Note that this has changed from prior years. The physical exam must be completed on or after June 15 to be valid for participation the following school year.

2. PCS Athletic Rules Sheet (signed and fees paid)

3. PCS Cooperative Sport Application and Commitment Form (signed)

Elementary and Junior High Football -- FM Athletics

For registration forms and information about football for elementary (grades 4-6) and junior high (grades 7-8) boys, please visit the FM Athletics website.

Important note: Please turn forms in to the PCS Office rather than mailing it to FM Athletics so that we can ensure our students are on the correct team(s).
